How do I file reimbursement claims for consultations at accredited hospitals?

Please follow these steps:

STEP 1: Download the reimbursement form from www.benefitsmadebetter.com (under the Downloadable Forms tab). Fill out the form completely.

STEP 2: Attach the required documents to your reimbursement form:

·      Original official receipt (with TIN)

·      Statement of Account from the accredited hospital

·      Medical certificate

·      Laboratory result (if applicable)

STEP 3: Send your completed reimbursement form and all required documents to your HR personnel as soon as possible. Please note that Intellicare must receive all your original documents within 30 days of your consultation.

Turnaround time: Intellicare will process reimbursement requests within 30 days of receiving all the required documents.
