What are the documents I need to upload to support the enrollment of my dependents?

For existing dependents: If you enrolled a dependent above 18 years old, you need to submit the Data Privacy Consent Forms with signature of your legal-age dependents. No need to submit other supporting documents.

For new dependents, you need to upload the following:

·      Husband/Wife - Marriage Certificate

·      Child/Children - Birth Certificate

·      Parent/s - Marriage Certificate

·      Sibling/s - Birth Certificate

·      Common law/Same-gender partner – HR Endorsement

·      If dependent’s surname differs from the employee’s – Employee’s Birth Certificate

·      If you enrolled dependent 18 years old and above - Data Privacy Consent Forms with signature of your legal-age dependents
