Intellicare reimbursement process 

IMPORTANT REMINDERS
  • As standard process, members are still required to submit the original (hard copy) official receipts for Intellicare to release the payment or check.
  • Members must file the reimbursement claims to Intellicare within 30 calendar days upon discharge or availment. Failure to file your reimbursements within this period will lead to disapproval of claims.
  • Intellicare’s turnaround time for processing of claims is 20 working days upon receipt of complete documents.
  • You may send your reimbursement follow ups and inquires to ActiveLink helpdesk.
Required Documents
  • Intellicare Reimbursement Form
  • Original Official Receipt with TIN
  • Hospital Statement of Account (both Detailed and Summary SOA)
  • Medical Certificate with Diagnosis
  • Laboratory results (if with diagnostic procedure)
  • Operative record with histopath (if with operation)
  • Police report and medico-legal report (if required)
 
Reimbursement Process
Online submission of claims is subject to evaluation, and original documents must be submitted.
 

1. Fill out the Intellicare Reimbursement Form. The form may be downloaded from your www.benefitsmadebetter.com account.

2. Submit the Intellicare Reimbursement form and required documents through Agora App or to ActiveLink Helpdesk.

3. Processing of filed reimbursement claims either in soft or hard copy will start the following day from the receipt of all requirements, which includes the principal member’s bank details and official receipt. Please be guided that you need to submit the hard copies of the Official Receipts to Intellicare as mandated by BIR.

4. If there are lacking documents, the turnaround time will reset upon receipt of complete documents.

5. Receipt of reimbursement claims can be done through online bank crediting or check pick-up at Intellicare’s office at GF, Feliza Building, VA Rufino Street, Legazpi Village, Makati City.

6. Bank information indicated in the member’s recent claims will supersede previous bank details. However, if no bank details will be provided, Intellicare will automatically use the previous bank details. Likewise, online bank crediting option will automatically apply if the member fails to put/tick the payment option in the submitted Intellicare Reimbursement Form.

7. Intellicare will provide a Lacking Advice Notice within 3 to 5 days if submitted claims have lacking details/information in the reimbursement form and/or lacking supporting documents. For non-coverable cases, a Denied Letter of Advice shall be released.

8. Submit all original, hard-copy and required documents to Intellicare. Members may submit the required documents below:
